[ANSWER: New software in use at the time cause the police report request to be routed incorrectly. The error was caught and the full version of the report was later provided. 10-19-2021]

[UPDATE: 12-18-2020 01:17 PM - The public version of the incident report had virtually no information to add. The address of the incident was listed. The deceased listed as Jaquell Mitchell-Cook. No information as to the cause of death. Somehow I found out about the incident nearly an hour before police did even thought the date and time of death is given as about 22 hours prior to my being told about it.]

12-03-2020 Does anyone know how 1 request for a police incident report could go missing and another missing but found a day later after being placed in a locked drop box at the Records Service Center?
- Both times the request was for an incident involving Jaquell Mitchell-Cook (Jerzey).
- Requests were dropped off in person in the lobby entrance at 1915 Olive St.
- When calling (314 444-5551) to check on the status:
- 1st request attempt (2 weeks after submitting as a default type request) - Was told I couldn't receive a copy of the report. There was no acknowledgement the request was received.
- Was transferred twice before speaking with someone.
- The woman sounded like someone I'd met before.
- Was told an incident occurred on 12-27-2020 at 12:27 am.
- Was told that someone had died/there was loss of life and gave her condolences.
- 2nd request attempt (5 days after submitting and specifying a Sunshine Law request) - Was told the request had not been received.
- Was told an incident occurred on 12-26-2020.
- Was told it was a theft.
- Thankfully was given a # for making the request again: 19-064174
- Received a call from Records today (12-02-2020) and was asked for clarification on what was being requested (the 2nd request attempt was received). I did not have the forms filled out correctly but the woman worked through fixing them.
- The incident (termed accident report) was found for # 19-064174.
- Called back to ask about bringing a money order there today and try to confirm which of the 2 dates given are correct (12-27-2020 or 12-26-2020).
- Was told it was a non-criminal suspicious circumstances incident on 12-26-2020 and someone had died/there was a loss of life.
- Will wait for the report.
- Was suggested I use the Online Records Portal instead of delivering requests in person.
